Hiring paperwork, explained
Truck Driver
Employment Application
A CDL-A driver employment application collects the driver's background, license details, driving experience, and work history so a carrier can begin its qualification review. It is the starting document—not the finished hiring file.
01 / A real document
Start with a clear picture of the driver.
This is the first page of the blank Driver Employment Application included in NadiaChicago Hiring Systems. It starts with applicant information, address history, and current commercial licenses or permits.
What comes after page one?
The complete application continues with the driver's experience, driving record, employment history, and certification. Ask the driver to complete the relevant sections and clarify missing or inconsistent information before you move on.
This image is a product preview. NadiaChicago's complete PDF and editable Word files are delivered with a purchased Hiring System; this page does not offer them as a free download.
02 / The information
What does the application collect?
Applicant and contact details
Identity, contact information, the position applied for, and address history.
Licenses and driving background
Commercial licenses or permits, endorsements, experience with equipment, and driving-record disclosures.
Work history
Previous employers, dates, driving duties, and information the carrier needs to follow up.
Certification
The driver's signed statement about the information supplied. A signature does not replace the carrier's review.
03 / The carrier's next steps
Received the application. Now what?
01Review it for completeness
Check dates, missing answers, license information, and gaps that need clarification. Keep a record of follow-up.
02Complete the applicable qualification checks
Review the current requirements for your operation, obtain the required records and consents, and document the results. The application does not order an MVR or run a Clearinghouse query.
03Organize supporting records
File the application with the appropriate qualification records. Keep sensitive business, tax, and payment information securely organized.
04Keep the file current
Track the follow-up dates that apply to the driver, such as license expiration and annual driving-record review. Assign responsibility for completing each action.
04 / One form, a larger file
An application is not a complete
Driver Qualification File.
The application records information supplied by the driver. It is one part of the carrier’s qualification file.
What else belongs in the DQ file?
The DQ file brings together the application and the qualification records the carrier must maintain, such as applicable motor vehicle records, road-test documentation or an accepted equivalent, medical qualification records, and the annual driving-record review.
A completed form alone does not show that the required checks have been performed or that a driver is ready for dispatch. Use the current DQ-file requirements in 49 CFR 391.51 (opens in a new tab) to check what applies to your operation.
05 / Put the next steps in order
Need more than the application?
Each $29 Hiring System includes ready-to-print PDFs, editable Word templates, employer guides in English and Russian, and the offline Hiring Workspace in one ZIP. Choose the packet that matches your actual working arrangement.

View the W-2 system →Owner-Operator · $29
The driver brings a truck, personally works under your carrier authority, and uses an equipment lease.
View the Owner-Operator system →Complete Bundle includes all three for $58. Three separate purchases cost $87, so you save $29. If you only need one system, it remains $29.
One-time purchase, no monthly subscription. The Workspace runs locally in your browser; you manage saved data and backups. It does not perform official checks or send email/SMS reminders while closed. This site does not collect completed driver applications.

A few practical questions
Is this an online application a driver can submit?
No. This page explains the document and shows a blank preview. Downloaded NadiaChicago forms are for your company's own process. Do not submit SSNs, CDL numbers, or other confidential driver information through this website.
Does the word “employment” mean this is only for W-2 drivers?
The title of the application does not decide the worker's status. Choose the rest of your paperwork around the working relationship your company has properly determined. A services agreement, employee onboarding packet, and equipment lease serve different purposes.
Do I have to buy a system to use FMCSA's example?
No. The official FMCSA sample linked above is a free, independent resource. NadiaChicago's paid systems add organized documents, employer guidance, and the offline Workspace.
